Cushing's Syndrome – Cortisol Excess / Excess Glucocorticoids
Cushing's syndrome refers to the totality of symptoms and physical changes that arise from a prolonged excess of cortisol or other glucocorticoids.
Cortisol is a vital hormone produced by the adrenal cortex. Among other things, it influences sugar, fat, and protein metabolism, blood pressure, the immune system, and the body's response to stress. Persistently high cortisol levels can affect numerous organs and metabolic processes.
How does Cushing's syndrome develop?
A distinction is generally made between exogenous and endogenous Cushing's syndrome.
The most common cause is long-term or high-dose treatment with **glucocorticoids ("cortisone") **, such as prednisolone, dexamethasone, or other cortisone preparations. This is known as exogenous or medication-induced Cushing's syndrome.
In endogenous Cushing's syndrome, the body itself produces too much cortisol. Possible causes include:

Cushing's disease, on the other hand, specifically refers to Cushing's syndrome caused by excessive ACTH production by the pituitary gland.
What symptoms can occur?
A long-standing cortisol excess can cause the following changes, among others:

If Cushing's syndrome is suspected, specific hormone tests are performed. These may include measurements of cortisol in blood, saliva, or urine, as well as dexamethasone suppression tests and ACTH determinations.
If an endogenous cortisol excess is confirmed, the underlying cause is then sought. Depending on the findings, imaging examinations of the pituitary gland or adrenal glands may be necessary.
Treatment depends on the cause. In medication-induced Cushing's syndrome, a medically supervised adjustment of glucocorticoid therapy may be possible. Cortisone preparations must not be discontinued independently or abruptly after prolonged use, as this can lead to dangerous cortisol deficiency.
In endogenous Cushing's syndrome, surgical interventions, medications, or other specialized therapies may be required.
